- ਅਰਜ਼ੀ ਵਿੱਚ ਦਰਜ ਸਾਰੇ ਪਰਿਵਾਰਕ ਮੈਂਬਰਾਂ ਦੀਆਂ ਪਾਸਪੋਰਟ ਸਾਈਜ਼ ਫੋਟੋਆਂ ਜਮ੍ਹਾਂ ਕਰੋ।
- ਸਰਕਾਰੀ ਸੰਸਥਾ/ਪ੍ਰੈਕਟੀਸ਼ਨਰ ਵੱਲੋਂ ਬਿਮਾਰੀ/ਅਪੰਗਤਾ ਦਾ ਸਰਟੀਫਿਕੇਟ
- ਜੇਕਰ ਘਰ ਵਿੱਚ LPG ਕੁਨੈਕਸ਼ਨ ਹੈ, ਤਾਂ LPG/ਗੈਸ ਬੁੱਕਲੈਟ ਦੀ ਕਾਪੀ।
- ਪਰਿਵਾਰ ਦੀ ਸਮੂਹਿਕ ਪਾਸਪੋਰਟ ਸਾਈਜ਼ ਫੋਟੋ
- ਮੌਤ ਦੇ ਸਰਟੀਫਿਕੇਟ, ਜੇਕਰ ਪਰਿਵਾਰ ਦਾ ਮੁਖੀ ਵਿਧਵਾ ਹੈ ਜਾਂ ਪਰਿਵਾਰ ਵਿੱਚ 21 ਸਾਲ ਤੋਂ ਘੱਟ ਉਮਰ ਦਾ ਕੋਈ ਅਨਾਥ ਬੱਚਾ ਹੈ।
- ਜੇਕਰ ਪਰਿਵਾਰ ਕੋਲ ਖੇਤੀਬਾੜੀ ਵਾਲੀ ਜ਼ਮੀਨ ਹੈ, ਤਾਂ ‘ਫਰਦ’

Who can apply
Permanent residents of Punjab State, excluding families where a member is a regular government employee of the Central or State Government—or their departments, boards, corporations, municipal committees, trusts, institutions, etc. (Note: Contractual, work-charged, outsourced, and daily-wage employees of these institutions are eligible to apply.)Households where combined annual income of all members from all sources is less than INR 800,000Punjab Smart Ration Card: ERCMS, AePDS & Online Apply :

Documents Need to Apply
- Aadhaar cards of all family members (mandatory – must be seeded and verified)
- Proof of residence in Punjab – (electricity bill, water bill, or rent agreement)
- Passport-size photograph of the head of household
- Mobile number (linked with Aadhaar)
- marriage certificate (For newly married spouse )
- birth certificate (For newborn)
- Old ration card number if converting from paper card to smart card
