West Bengal Mukhyamantri Swasthya Bima Yojana: Cashless Treatment up to ₹5 Lakh for Families
A hospital stay can wipe out a family’s savings in a matter of days. The West Bengal Mukhyamantri Swasthya Bima Yojana (MMSBY) is a health insurance scheme designed to prevent that for families who are not covered by the central Ayushman Bharat scheme. It was launched in the state on 16 August 2026.
A name change to know about
MMSBY is the renamed version of the earlier Swasthya Sathi Scheme, which the current government has renamed. If you already hold a Swasthya Sathi card or URN (the family’s unique reference number), you are likely part of the group this scheme is built around.
The introduction describes the scheme as “proposed” in the highlights but “now operational” in the main text. The launch date and the working enrolment process indicate that it is running, but if you are unsure of your own status, use the eligibility check below.
What the scheme offers
- Cashless cover of up to ₹5 lakh per family every year
- Treatment at all hospitals empanelled under Ayushman Bharat PM-JAY, with portability across India
- Free individual health cards
- No limit on family size under the family floater plan
- Cover for pre-existing diseases from day one, with no waiting period
You pay no premium, and the aim is to protect families against hospitalisation costs without them paying eligible expenses upfront.
Who is eligible
- The applicant must be a permanent resident of West Bengal.
- The applicant or family should be covered under the Swasthya Sathi Scheme.
- The family should not be covered under Ayushman Bharat PM-JAY.
Who is not eligible
- Families with an annual income above ₹8 lakh
- Employees or pensioners covered under the West Bengal Health Scheme, 2008
- Employees or pensioners covered under CGHS or the ESI Scheme
- Employees or pensioners covered under any group medical insurance scheme run by Central or State PSUs, urban local bodies, statutory bodies, corporations, trusts, societies, boards or similar government-controlled organisations
- State Government, Central Government or parastatal employees and pensioners who receive a medical allowance under government rules
- Family members above 70 who are covered under the Ayushman Vay Vandana Scheme
- SIR-deleted names, including absentee, deceased, shifted and duplicate records
- Any other category the state notifies from time to time
Step one: check your status
The easiest first step is the Aadhaar-based eligibility check:
- Visit the West Bengal State Health Agency’s official portal.
- Go to Citizen Services and select PM-JAY Eligibility Check.
- Enter your Aadhaar number.
- If your Aadhaar is linked to a ration card in the AAY, SPHH or PHH category, the portal shows your AB-PMJAY status. If it is linked to the RKSY1 category, it shows your MMSBY status. If checks are still under way, it will say the case is under verification.
If your Aadhaar is not found under any ration card category but you have a valid Swasthya Sathi URN, enter the URN on the same portal. An OTP goes to the registered mobile number of the main Swasthya Sathi beneficiary. After verification you see a form listing every family member and their current AB-PMJAY or MMSBY status. If anyone lacks an ABHA ID, you will need to enter that person’s Aadhaar number to generate one.
Step two: how enrolment works
There are three routes.
Route 1: field verification (the main route). Families in the RKSY1 category of the NFSA ration card database are identified, and Field-Level Workers physically verify pre-filled forms. The forms are then digitised and checked by Data Entry Operators and Extension Officers. Once verified, the health card (also called the Swasthya Sathi Card) is activated for the family. Families in this group do not need to apply on their own.
Route 2: online application. This is only for Swasthya Sathi URN holders whose Aadhaar is not found in the AAY, SPHH, PHH or RKSY1 databases:
- Visit the State Health Agency portal and select MMSBY Enrollment under Citizen Services.
- Enter your Aadhaar number and the OTP sent to your registered mobile.
- Check the family details and statuses shown. Add Aadhaar numbers where an ABHA ID is missing.
- If the option appears, submit the online application.
- It goes to a Field-Level Worker for verification, then to an Extension Officer for digitisation and verification, and the Extension Officer marks you eligible or ineligible.
Route 3: through a hospital. At a hospital empanelled under Swasthya Sathi, give your existing Swasthya Sathi URN and beneficiary details. The hospital can digitise your family’s details during treatment, and they are then processed and verified for MMSBY enrolment. This is useful if you need care before your card is activated, but it is wise to confirm with the hospital before admission that this applies to your situation.
Documents
Keep ready the Aadhaar cards of the applicant and family members, the family’s registered mobile number, family or household details, identity and address proof, your existing Swasthya Sathi card (if any), AB-PMJAY information or card (if any), and any other document the competent authority prescribes.
Questions the source does not answer
The source lists a few questions without giving the answers, such as the exact annual income cap for every group. Its ineligibility list does state that families above ₹8 lakh a year are excluded, so that is the figure to rely on, but confirm it with the department if your income is close to the line. It also does not give a last date for enrolment.
Where to get help
- MMSBY toll-free number: 18003455384
- MMSBY helpdesk email: the source prints it as shawb.pmjay@gmail,com, with a comma, so the address is probably shawb.pmjay@gmail.com. Check the official portal before relying on it.
